Rescheduled and Review Classes
Please submit the appropriate form for your class request after reviewing the information below. Any rescheduled or review classes that must meet via Zoom should be held using the Zoom Conferencing link in Canvas, and are subject to approval from the Office of Academic Affairs. It is the responsibility of the professor to share the class change information with the students using a Canvas announcement or email message.
Rescheduled Class Form
Common Primary Uses:
- Used to schedule a make-up class
Common Reservation Requirements:
- For faculty and staff use only
- Copies of these forms are sent to the Office of Academic Affairs and the Information Systems Technology Service Desk
Review Class/ Class Extension Request Form
Common Primary Uses:
- Used to schedule a review class
- Used to request an extension to an originally scheduled class time
- Used to request break out rooms
Common Reservation Requirements:
