Registrar's Certification Form
Because the LSAC Document Assembly Service does not serve to authenticate transcripts, all students must complete a Registrar's Certification Form, with the following exception: Internationally-educated students who use the International Transcript Authentication and Evaluation Service are NOT required to complete a Registrar's Certification Form.
All U.S. applicants are required to submit this form whether or not they are using the LSAC service.
We only require this form to be filled out by the applicant's law degree granting institution (or undergraduate degree granting institution for MSL applicants). We do not need this form from the applicant's undergraduate institution unless they are an MSL applicant.
Applicants should complete the top portion of the form. The bottom portion of the form should be completed by the appropriate university official. The form should be sent directly to the Office of Admissions from the university official's office. The Office of Admissions will accept the Registrar's Certification Form electronically. Applicants may have the completed form e-mailed to firstname.lastname@example.org or faxed to (202)662-9439.
